Many women consider breast augmentation surgery to add volume, lift and enhance the overall appearance of their breasts. Because of traditional post-surgical healing times and recovery, some women are unable to undergo breast augmentation. Recovery from traditional breast augmentation surgery can take up to three weeks and requires limitations including restrictions from lifting, restrictions to daily moving and restrictions to working. Recovery from traditional breast augmentation also involves the use of prescription pain killers. The use of prescription pain killers may not be a viable choice for all women.

What is pain-free breast augmentation?

Breast augmentation is a surgical procedure that modifies, adds volume and enhances the shape and form of the breasts. Because of aging, weight gain and loss, natural body proportions, deformities or injury, some women wish to augment the appearance, size, and shape of their breasts. Breast augmentation surgery is a surgical procedure involving adding volume and size to the breasts resulting in a more voluptuous, toned and natural appearance.

Traditionally, breast augmentation surgery required general anesthesia or deep intravenous sedation and sizeable incisions around the breasts. The sedation combined with the large incisions was cause for a long recovery period with limitations to daily living. Over the past few years, surgeons have developed innovative techniques that enable a decrease in the amount of sedation used and the size of the incisions, drastically reducing the recovery time and limitation.

The main difference between traditional breast augmentation surgery and pain free breast augmentation, is the anesthetic used. Traditional surgeries required the use of strong anesthetic and deep sedation, causing drowsiness and pain during the recovery period.

In pain-free breast augmentation, surgeons use a local anesthetic (Depofoam Bupivacaine, also known as Exparel), is the latest in long – lasting local anesthetic. Depofoam bupivacaine is a derivative of Marcaine but in a time released form administered by injection into the site at the time of surgery. Depofoam Bupivacaine has been widely used for foot surgery because of its efficiency and ease of use.

Depofoam bupivacaine is a new formulation of a well-established drug: bupivacaine.

The manufacturers have developed this new formula by inserting the drug into small carriers (balloons) known as liposomes. Once injected, the liposomes slowly dissolve and release a small amount of the drug gradually over the next one to three days. Depofoam bupivacaine has an extended half-life. The half-life of a drug is the amount of time it takes for the drugs to release and be reduced by one-half. This extended half-life is responsible for the long length of numbing effects provided by depofoam bupivacaine.

Depofoam bupivacaine provides numbing to the incisions for one to three days. The ability of depofoam bupivacaine to numb the site for such a long period of time drastically decreases the pain associated with recovery, the need for prescription pain killers and the recovery time post-surgery.

Given the numbing abilities of depofoam bupivacaine, and the smaller incisions, there is very minimal pain post-surgery that is treated with over the counter non-steroid anti-inflammatory drugs such as ibuprofen. The absence of pain at the incision sites makes for a much faster recovery with minimal limitations on daily activities.
Because the incisions used in pain-free breast augmentation are drastically smaller than traditional breast augmentation surgeries, the amount of depofoam bupivacaine injected is also very small. The injection of depofoam bupivacaine is done in the muscle and slowly released.

Additionally, depofoam bupivacaine is not a narcotic, so the traditional effects generally felt by patients post-surgery (constipation, itching and drowsiness) do not occur post-surgery.

Procedure

Prior to surgery you will meet with Dr. Craft and your enhancement team will work with you to identify problem areas and create an enhancement plan. Depending on the enhancement you desire, small incisions will be placed around and under your breasts.

The locations of the incisions may include:

  • Inframammary: Underneath your breasts (inframammary)
  • Transaxillary: Under your arms
  • Periareolar: Around the nipple
  • Scar: Incisions may also be made through scars, such as a mastectomy scar

During surgery, you will be administered either general anesthesia or an intravenous sedation. Then Dr. Craft will make small incisions in the necessary areas. Once the incisions are made, the implant will be inserted and placed. Next, Dr. Craft will inject depofoam bupivacaine directly into the incisions site and around the implant.

Depofoam bupivacaine will begin working immediately to numb the site and will continue to numb post-surgery and during recovery for one to three days.

Recovery & Pain

Recovery Process

Recovering from pain-less breast augmentation takes much less time and involves much less interventions than recovery from traditional breast augmentation. The general recovery time is two to three days.

 Your enhancement team will provide you with specific post-surgery recovery instructions, but generally:

  • Expect to feel some short-lived grogginess immediately after surgery.
  • Expect several small incisions around your breasts.
  • Expect to keep activities and daily life movements to a minimum.
  • Expect very minimal swelling and/or bruising around the site.
  • Your team may advise breast massage and other techniques post-surgery.

Pain

Most women do not report any pain post-surgery. Some women do report mild discomfort that is treated with over the counter non-steroid anti-inflammatory medication such as ibuprofen every four to six hours. It is important not to use any other prescription pain medication without first speaking to your surgeon.
